Support Coordination Level 1 & 2

Through Support Coordination Level 1 & 2 we assist clients to seek assistance from a Support Coordinator with Further qualifications/experience required to strengthen a participant’s ability to design and the build their supports with an emphasis on linking the broader systems of support across a complex service delivery environment. Coordination of Supports is to focus on supporting participants to direct their lives, not just their services. This may include resolving points of crisis, and developing resilience in the participant's network.

Therapy Assistance - Level 2

Unique Traits Community Services can provide participants with therapeutic support by an allied health assistant working under the delegation and supervision of a therapist, where the therapist is satisfied that the allied health assistant is able to work independently without direct supervision at all times.

Nutrition and Dietetics

Unique Traits Community Services is committed to supporting individuals with nutrition and dietetics by providing personalised guidance and resources tailored to each person's unique dietary needs and health goals. Through a holistic approach, our team of qualified nutritionists and dietitians work closely with individuals to assess their current eating habits, nutritional requirements, and any underlying health concerns, such as chronic conditions or allergies. We offer individualised nutrition plans that promote overall health, improve well-being, and address specific needs like weight management, diabetes, heart health, or digestive issues.

Individual Counselling

This funding is specifically designated to provide access to personalised, one-on-one counselling sessions with a highly qualified and experienced counsellor. Unique Traits Community Services offers individuals the opportunity to engage in a safe, confidential, and supportive environment where they can explore personal challenges, work through emotional or psychological concerns, and gain valuable insights into their mental health and well-being.

Speech Pathology

Unique Traits Community Services offers specialised support for individuals through Speech Pathology services, aimed at improving communication skills and addressing a wide range of speech, language, and swallowing difficulties. Our team of qualified speech pathologists works closely with individuals to assess their specific needs, whether they relate to articulation, fluency, language comprehension, expressive communication, voice disorders, or social communication challenges.

Training for Carers and Parents

Unique Traits Community Services offers specialised training and support for carers and parents to equip them with the skills, knowledge, and confidence needed to provide the best care for individuals with diverse needs. Our training programs are designed to empower carers and parents to effectively support the physical, emotional, and social well-being of those in their care, whether they are managing disabilities, mental health challenges, aging-related needs, or other complex conditions.

We provide tailored, evidence-based training that covers a wide range of topics, including behavior management, communication strategies, personal care techniques, understanding medical conditions, and fostering independence in daily activities. This training helps carers and parents develop a deeper understanding of the specific needs of the individual they support, enabling them to provide care that is both compassionate and effective

Home Maintenance

Unique Traits Community Services provides essential home maintenance support to help individuals maintain a safe, functional, and comfortable living environment. We understand that home upkeep can be challenging for individuals with physical limitations, aging-related issues, disabilities, or those simply needing extra assistance, and our services are designed to ensure that individuals can continue to live independently in their own homes without the stress of managing routine maintenance tasks

Gardening

Unique Traits Community Services provides tailored gardening support to help individuals create and maintain beautiful, functional outdoor spaces, promoting both physical and mental well-being. We recognise that gardening can be a therapeutic and rewarding activity, but for some individuals—whether due to age, disability, or health challenges—it may be difficult to manage independently. Our gardening services are designed to meet the specific needs of each person, ensuring that they can enjoy the benefits of a well-maintained garden without the stress or difficulty of managing the physical demands on their own.

Community Access

Unique Traits Community Services is dedicated to supporting individuals with Community Access by helping them fully participate in their local communities, fostering independence, and building meaningful connections. We understand that for many, access to social, recreational, and professional opportunities can be limited due to various barriers, such as mobility issues, lack of support, or social isolation. Our services aim to break down those barriers by providing personalised assistance, advocacy, and resources to ensure individuals can engage in community activities and services that align with their interests and needs.
We can also assist participants with weekends and after hour supports.

Domestic Assistance

When subcontracting our services, partner organizations can access the expertise and resources of our skilled team, allowing them to meet the diverse needs of their clients without overextending their own capacities. Our services are flexible, and we work collaboratively with partners to integrate our offerings into their existing frameworks, ensuring seamless delivery of care. Group Counselling
